Please note: the application is a very important step to my services! This pre-consultation form allows me to gather enough information to better understand my future clients. I must not only take into consideration your skin type/conditions and/or lash history prior to your appointment, but most importantly your concerns or any possible allergies I should be aware of, as well as anything that might stand out to me as a contraindication. In addition, I ask my potential clients to keep in mind the time, resources, and at-home care it may require to achieve certain desired looks given the current condition of their skin or lashes.
